ABSTRACT

ABSTRACT Background: To assess the efficacy of applying the endoscopic reference score for EoE (EREFS) in children with symptoms of esophageal dysfunction naïve to proton pump inhibitor (PPI) therapy. Methods: An observational cross-sectional study was conducted by reviewing reports and photographs of upper gastrointestinal endoscopies (UGE) and esophageal biopsies of patients with symptoms of esophageal dysfunction. Patients who were treated with PPI or had other conditions that may cause esophageal eosinophilia were excluded. Results: Of the 2,036 patients evaluated, endoscopic findings of EoE were identified in 248 (12.2%) and more than one abnormality was observed in 167 (8.2%). Among all patients, 154 (7.6%) presented esophageal eosinophilia (≥15 eosinophils per high power field) (P<0.01). In this group, 30 patients (19.5%) had normal endoscopy. In patients with EoE, edema (74% vs 6.5%, P<0.01) and furrows (66.2% vs 2.4%, P<0.01) were more prevalent than in the control group. Association of edema and furrows was more frequent in patients with EoE than in the control group (29.2% vs 1.6%, P<0.01, OR=24.7, CI=15.0-40.5). The presence of more than one endoscopic finding had sensitivity of 80.5%, specificity of 93.4%, positive predictive value (PPV) of 50%, negative predictive value (NPV) of 98.3%, and accuracy of 92.4%. Conclusion: In conclusion, this study showed that endoscopic features suggestive of EoE had high specificity and NPV for diagnosing EoE in children naïve to PPI therapy. These findings highlight the importance of the EREFS in contributing to early identification of inflammatory and fibrostenosing characteristics of EoE, making it possible to identify and to avoid progression of the disease.


RESUMO Contexto: Avaliar a eficácia da aplicação do escore de referência endoscópico para EoE (EREFS) em crianças com sintomas de disfunção esofágica sem tratamento prévio com inibidores da bomba de prótons (IBP). Métodos: Foi realizado um estudo transversal observacional por meio de revisão de laudos e fotos de endoscopia digestiva alta (EDA) e biópsias de esôfago de pacientes com sintomas de disfunção esofágica. Pacientes tratados com IBP ou com outras condições que podem causar eosinofilia esofágica foram excluídos. Resultados: Dos 2.036 pacientes avaliados, os achados endoscópicos de EoE foram identificados em 248 (12,2%) e mais de uma anormalidade foi observada em 167 (8,2%). Entre todos os pacientes, 154 (7,6%) apresentaram eosinofilia esofágica (≥15 eosinófilos por campo de grande aumento) (P<0,01). Nesse grupo, 30 pacientes (19,5%) apresentaram endoscopia normal. Em pacientes com EoE, edema (74% vs 6,5%, P<0,01) e linhas verticais (66,2% vs 2,4%, P<0,01) foram mais prevalentes quando comparados ao grupo controle. A associação de edema e linhas verticais foi mais frequente em pacientes com EoE do que no grupo controle (29,2% vs 1,6%, P<0,01, OR=24,7, IC=15,0-40,5). A presença de mais de um achado endoscópico teve sensibilidade de 80,5%, especificidade de 93,4%, valor preditivo positivo de 50%, valor preditivo negativo de 98,3% e acurácia de 92,4%. Conclusão: Em conclusão, esse estudo mostrou que as características endoscópicas sugestivas de EoE apresentam especificidade e VPN elevados para o diagnóstico da enfermidade em crianças sem tratamento prévio com IBP. Estes achados reforçam a importância do EREFS em contribuir para a identificação precoce de características inflamatórias e fibroestenosantes, possibilitando identificar e evitar a progressão da doença.

Las sibilancias recurrentes del preescolar son un problema prevalente. 50% de todos los niños tiene al menos un episodio de sibilancias en los primeros 6 años. Sin embargo, solo 4 % de los menores de 4 años tiene diagnóstico de asma. Por este motivo es fundamental realizar una adecuada anamnesis y examen físico tendientes a descartar causas secundarias, lo que debe ser complementado con exámenes de laboratorio de acuerdo con la orientación clínica. En la actualidad se recomienda indicar tratamiento de mantención con corticoides inhalados en aquellos niños que tengan episodios repetidos de obstrucción bronquial y que tengan una alta probabilidad de respuesta favorable a esta terapia. Se ha demostrado que aquellos pacientes que tienen un recuento de eosinófilos en sangre > 300 células por mm3 o aquellos que presentan una prueba cutánea positiva o IgE específicas positivas para alérgenos inhalados, responderán adecuadamente al tratamiento con esteroides inhalados.


Recurrent wheezing in preschoolers has a high prevalence. 50% of all children have at least one wheezing episode in the first six years of life. However, only 4% of children under four years of age are diagnosed with asthma. Therefore, it is essential to carry out an adequate medical history and physical examination to rule out secondary causes, which must be complemented with laboratory tests in accordance with clinical guidance. It is recommended to indicate maintenance treatment with inhaled corticosteroids to those children who have repeated episodes of wheezing and who have a high probability of a good response to this therapy. It has been demonstrated that those patients who have blood eosinophil count > 300 cells per mm3 or those who have a positive skin test or positive specific IgE for inhaled allergens will have a good response to inhaled corticosteroids.

ABSTRACT

Objective:To explore the risk factors of acute exacerbation (AE) of chronic obstructive pulmonary disease (COPD) and whether Eosinophil (EOS) in peripheral blood can guide the treatment of inhaled corticosteroids (ICS).Methods:This study was a single center, Prospective cohort study. The subjects of this study were from stable COPD patients who were treated in the Department of Respiratory Medicine of the Xiangya Second Hospital of Central South University from January 2020 to December 2021. Patient general information, past year AE status, exposure risk factors, modified version of the British Medical Research Council Respiratory Difficulty Questionnaire (mMRC) score, Chronic Obstructive Pulmonary Disease Assessment Questionnaire (CAT) score, ICS usage, lung function, blood routine, etc. were collected. We followed up the patient for one year. During the follow-up period, the clinical characteristics of patients with and without AE were compared to analyze the correlation between blood EOS and ICS use.Results:The median blood EOS of 617 stable COPD patients was 0.13×10 9/L, 289 patients (46.8%) with chronic obstructive pulmonary disease had a history of AE, and 207 patients (33.5%) experienced AE during 1-year follow-up. The results of univariate analysis showed that the future occurrence of AE in COPD was correlated with body mass index (BMI), AE history, Global Initiative for Chronic Obstructive Lung Disease (GOLD) grading, GOLD grouping, mMRC score, and CAT score (all P<0.05). The results of logistic multiple factor regression analysis showed that patients with BMI<24 kg/m 2, AE in the past year, severe AE in the past year, smoking history and other exposure factors, GOLD level 2 or above, GOLD C and D groups, and mMRC score≥ 2 had a higher risk of future AE (all P<0.05). There was no statistically significant difference in the incidence of AE between patients with COPD with different levels of EOS and those without ICS during a 1-year follow-up period (all P>0.05). Conclusions:The past 1-year AE history, BMI, exposure risk factors, degree of airflow restriction, and respiratory symptoms of patients with chronic obstructive pulmonary disease can predict future AE risk. There is no significant difference in future AE risk among patients with different levels of EOS, and EOS cannot guide ICS treatment to reduce AE risk.

ABSTRACT

Objective:To investigate the distribution of the total IgE, and their relationship with allergens and peripheral blood eosinophils in patients with allergic.Methods:The cross-sectional study was used, and 1 417 patients with allergic in Shanghai General Hospital, Shanghai Jiao Tong University School of Medicine were selected. The serum total IgE was detected by enzyme-linked immunosorbent assay (ELISA), and the total IgE>60 kU/L was increased. The blood routine was detected by automated hematology analyzer. Using inhalant allergens and food allergens specific IgE antibody detection kits, the serum or plasma allergen-specific IgE antibodies were detected by Oumeng imprinting in vitro semi quantitative method.Results:Among 1 417 patients with allergic, elevated total IgE was in 617 cases (43.54%), normal total IgE in 800 cases (56.46%); 749 cases (52.86%) were allergic, the most common inhalation allergen was dust mite (38.72%, 218/563), and the most common food allergen was peanut (24.01%, 109/454). The rate of normal total IgE, eosinophils and non allergic was 19.20% (272/1 417). The male proportion, ratio of eosinophils, eosinophils count, total index of inhaled allergens, total index of non inhaled allergens, number of positive allergens, total positive index and average positive index in patients with elevated total IgE were significantly higher than those in patient with normal total IgE, the age was significantly lower than that in patients with normal total IgE, and there were statistical differences ( P<0.01). The total IgE<60 kU/L (normal total IgE) was in 800 cases, total IgE from 60 to 499 kU/L (mild elevation of total IgE) in 487 cases, total IgE from 500 to 999 kU/L (moderate elevation of total IgE) in 78 cases, total IgE≥1 000 kU/L (severe elevation of total IgE) in 52 cases. The male proportion and age in patients with sever elevation of total IgE and moderate elevation of total IgE were significantly higher than those in patients with mild elevation of total IgE and normal total IgE, and there were statistical differences ( P<0.05). The compound allergy rate in patients with severe elevation of total IgE was significantly higher than that in patients with moderate elevation of total IgE, mild elevation of total IgE and normal total IgE: 73.08% (38/52) vs. 60.26% (47/78), 38.40% (187/487), 17.00% (136/800), and there was statistical difference ( P<0.05). The ratio of eosinophils, eosinophils count and number of positive allergens increased with increasing total IgE level, and there was statistical difference ( P<0.05). The number of dermatophagoides/dermatophagoides culinae allergic in patients with moderate elevation of total IgE was significantly more than that in patients with sever elevation of total IgE, mild elevation of total IgE and normal total IgE, and there was statistical difference ( P<0.05). Conclusions:Allergic disease may show a normal level of total IgE. Males are more likely to be troubled with high level of IgE. Patients with total IgE ≥1 000 kU/L should be alert to the possibility of poly-sensitization. Total IgE was no longer correlated with individual allergy severity in patients with total IgE≥500 kU/L.

ABSTRACT

Objective:To investigate the predicting value of eosinophil-to-neutrophil ratio (ENR) for outcomes at 3 months after intravenous thrombolysis in patients with acute ischemic stroke (AIS).Methods:Patients with AIS received intravenous thrombolysis in the Department of Neurology, Huai'an First People's Hospital from July 2019 to July 2022 were included retrospectively. Multivariate logistic regression model was used to determine the independent correlation between ENR and outcomes at 3 months after intravenous thrombolysis. The receiver operating characteristics (ROC) curve was used to evaluate the predictive value of ENR levels for poor outcomes at 3 months after intravenous thrombolysis. Results:A total of 352 patients with AIS receiving intravenous thrombolysis were enrolled, including 240 men (68.1%), age 66.46±12.00 years old. The median National Institutes of Health Stroke Scale score was 8 (interquartile range, 5-13). At 3 months after onset, 215 patients (61.0%) had good outcomes, 137 (38.9%) had poor outcomes. Univariate analysis showed that the median ENR×10 2 level of the poor outcome group was significantly lower than that of the good outcome group ( Z= –7.305, P<0.01). Multivariate logistic regression analysis showed that lower ENR×10 2 was an independent risk factor for poor outcomes at 3 months after intravenous thrombolysis (odds ratio 0.619, 95% confidence interval 0.514-0.745; P<0.01). ROC curve analysis showed that the area under the curve for ENR×10 2 predicting the poor outcomes after intravenous thrombolysis was 0.731 (95% confidence interval 0.678-0.784; P<0.01). The optimal cutoff value was 0.625 and the corresponding sensitivity and specificity were 94% and 40%, respectively. Conclusion:Lower ENR before intravenous thrombolysis in patients with AIS is independently associated with the poor outcomes at 3 months.

ABSTRACT

Eosinophilic gastrointestinal diseases are a group of diseases with repeated or persistent gastrointestinal symptoms and the increase of eosinophils in gastrointestinal mucosa.Pathology shows an increase in the number of eosinophils in gastrointestinal mucosa.Fibrosis can be seen in the lamina propria of esophageal mucosa in patients with eosinophilic esophagitis.A variety of cytokines may be chemotactic to the aggregation of eosinophils, including Th2 cytokines, eotaxin, thymic stromal lymphopoietin, macrophage migration inhibitory factor, sialic acid-binding immunoglobulin-like lectin, integrin and extracellular matrix protein.The intestinal tissue injury of eosinophilic gastrointestinal diseases may be related to eosinophil degranulation and secretion of specific products, inflammatory response, oxidative damage, fibrosis, tissue remodeling and impaired barrier function.

ABSTRACT

OBJECTIVE@#To investigate the prevalence of vitamin D deficiency and its association with blood eosinophil count in healthy population and patients with chronic obstructive pulmonary disease (COPD).@*METHODS@#We analyzed the data of a total 6163 healthy individuals undergoing routine physical examination in our hospital between October, 2017 and December, 2021, who were divided according to their serum 25(OH)D level into severe vitamin D deficiency group (< 10 ng/mL), deficiency group (< 20 ng/mL), insufficient group (< 30 ng/mL) and normal group (≥30 ng/mL). We also retrospectively collected the data of 67 COPD patients admitted in our department from April and June, 2021, with 67 healthy individuals undergoing physical examination in the same period as the control group. Routine blood test results, body mass index (BMI) and other parameters were obtained from all the subjects, and logistic regression models were used to investigate the association between 25(OH)D levels and eosinophil count.@*RESULTS@#The overall abnormal rate of 25(OH)D level (< 30 ng/mL) in the healthy individuals was 85.31%, and the rate was significantly higher in women (89.29%) than in men. Serum 25(OH)D levels in June, July, and August were significantly higher than those in December, January, and February. In the healthy individuals, blood eosinophil counts were the lowest in severe 25(OH)D deficiency group, followed by the deficiency group and insufficient group, and were the highest in the normal group (P < 0.05). Multivariable regression analysis showed that an older age, a higher BMI, and elevated vitamin D levels were all risk factors for elevated blood eosinophils in the healthy individuals. The patients with COPD had lower serum 25(OH)D levels than the healthy individuals (19.66±7.87 vs 26.39±9.28 ng/mL) and a significantly higher abnormal rate of serum 25(OH)D (91% vs 71%; P < 0.05). A reduced serum 25(OH)D level was a risk factor for COPD. Blood eosinophils, sex and BMI were not significantly correlated with serum 25(OH)D level in patients with COPD.@*CONCLUSION@#Vitamin D deficiency is common in both healthy individuals and COPD patients, and the correlations of vitamin D level with sex, BMI and blood eosinophils differ obviously between healthy individuals and COPD patients.

ABSTRACT

Objective:To explore the clinical correlation between peripheral blood basophil levels and chronic sinusitis (CRS) subtypes. Methods:One hundred and twenty-six patients with CRS and 103 healthy cases from physical examination admitted to the First Affiliated Hospital of Soochow University from January 2021 to October 2022 were retrospectively analyzed. According to the histopathological classification, CRS patients were divided into eosinophilic chronic sinusitis (eCRS) group (47 cases) and non eosinophilic chronic sinusitis (non-eCRS) group (79 cases). The differences among the three groups in peripheral blood inflammation cell counts, eosinophils-to-basophils ratio(bEBR), basophils-to-neutrophils ratio(BNR), basophils-to-lymphocytes ratio(BLR), basophils-to-monocytes ratio(BMR) were compared, and study the correlation between each index and Lund-Mackay score, and the correlation between basophils in peripheral blood and other inflammatory cells. Results:The counts of basophils in the peripheral blood of the healthy control group, eCRS group and non-eCRS group were 0.03±0.01, 0.04±0.02, 0.03±0.02, respectively, the eosinophils-to-basophils ratio(bEBR) were 5.64±4.22, 8.38±5.95, 4.55±3.90, the basophils-to-neutrophils ratio(BNR) were 0.01±0, 0.01±0.01, 0.01±0.01, and the basophils-to-lymphocytes ratio(BLR) were 0.01±0.01, 0.02±0.01, and 0.02±0.01, respectively, the basophils-to-monocytes ratio(BMR) were 0.08±0.04, 0.11±0.06, and 0.08 ±0.04 respectively. There was a statistically significant difference between eCRS group and healthy control group, non-eCRS group(P<0.01), while there was no statistically significant difference between non-eCRS group and healthy control group(P>0.05). Basophil counts (r=0.185 5, P<0.05), BLR(r=0.226 9, P<0.05), BMR(r=0.228 1, P<0.01) in patients with CRS were positively correlated with Lund Makey score. In addition, basophils were also positively correlated with eosinophils(r=0.479 2, P<0.01), lymphocytes(r=0.259 4, P<0.01), and monocytes(r=0.256 4, P<0.01) in patients with CRS. Conclusion:The peripheral blood basophil count, BLR and BMR were significantly increased in eCRS, and were significantly positively correlated with Lund -Makey score. It has the potential to develop into disease biomarkers and new therapeutic targets of eCRS.

Abstract Objective: Eosinophilic Chronic Rhinosinusitis (ECRS) is refractory and recurrent, requiring long-term follow-up after Endoscopic Sinus Surgery (ESS). Endoscopic evaluation is common during postoperative assessment, but how the findings vary over time in postoperative ECRS patients who were treated by the recent standardized management is unclear. We assessed the long-term change in postoperative endoscopic score in ECRS patients using a novel endoscopic scoring system (Escore). Methods: This retrospective study included 80 ECRS patients who underwent full-house ESS and were followed for longer than 1-year. Endoscopic procedures were repeated at every follow-up visit and postoperative Escores were assessed from 3-months up to 5-years (median follow-up period was 3-years). Results: The Escore did not significantly change from that at 3-months (3m-Escore). The Escore at the final observation point (f-Escore) among 80 patients was 9.2 ± 5.6 and there was no significant difference from the 3m-Escore (8.5 ± 4.1, p = 0.363). Twenty-one patients (21/80, 26.3%) were considered to have endoscopically uncontrolled ECRS at their final observation points and the 3m-Escore was identified as an independent predictive factor by multivariate logistic regression analysis. The 3m-Escore cut-off value that was able to predict endoscopically uncontrolled ECRS after long-term follow-up was ≥12. Conclusion: We demonstrated the long-term endoscopic outcomes after full-house ESS and continuous outpatient treatment. Early endoscopic findings (3m-Escore) were a potential predictive factor for the later endoscopic outcome, and a 3m-Escore of 12 or higher may be an indicator of the poor long-term prognosis of sinus mucosa. Abstract Objective: The aim of this study was to identify clinical and complete blood count differences between pediatric hospitalized patients with sickle cell disease infected or not by SARS-CoV-2 and compare the complete blood count of patients with sickle cell disease infected by SARS-CoV-2 before hospitalization and on admission. Methods: This study was a single-center prospective cohort. Data were collected from medical records of pediatric inpatients with sickle cell disease under 18 years old infected or not with SARS-CoV-2 from the first visit to the hospital until discharge and from the last medical appointment. All patients were tested for SARS-CoV-2 by the real-time reverse transcription polymerase chain reaction. Results: Among 57 pediatric patients with sickle cell disease hospitalized from March to November 2020 in a Brazilian academic hospital, 11 (19.3%) had a positive result for SARS-CoV-2. Patients infected by SARS-CoV-2 had a higher prevalence of comorbidities than the ones who were not infected (63.6 vs. 30.4%; p=0.046). During hospital stay, no clinical or complete blood count differences between groups were found. There was a decrease in eosinophil count on hospital admission in patients with sickle cell disease infected by SARS-CoV-2 (p=0.008). Conclusions: Pediatric hospitalized patients with sickle cell disease infected by SARS-CoV-2 had more comorbidities and had a decrease in eosinophil count between hospital admission and the last medical appointment.


RESUMO Objetivo: Identificar diferenças clínicas e laboratoriais entre pacientes pediátricos hospitalizados com doença falciforme infectados ou não por SARS-CoV-2 e comparar o hemograma completo de pacientes com doença falciforme infectados por SARS-CoV-2 antes da hospitalização e durante a admissão. Métodos: Coorte prospectiva unicêntrica, cujos dados foram coletados em prontuários de pacientes pediátricos internados com doença falciforme, menores de 18 anos, infectados ou não com SARS-CoV-2, desde a primeira visita ao hospital até a alta e desde a última consulta médica. Todos os pacientes foram testados para SARS-CoV-2 pela transcrição reversa seguida de reação em cadeia da polimerase em tempo real. Resultados: Dos 57 pacientes pediátricos com doença falciforme internados de março a novembro de 2020 em um hospital universitário brasileiro, 11 (19,3%) apresentaram resultado positivo para SARS-CoV-2. Pacientes infectados pelo SARS-CoV-2 apresentaram maior prevalência de comorbidades do que aqueles não infectados (63,6 vs. 30,4%; p=0,046). Durante a internação hospitalar, não foram encontradas diferenças clínicas ou laboratoriais entre os grupos. Houve diminuição da contagem de eosinófilos na admissão hospitalar em pacientes com doença falciforme infectados pelo SARS-CoV-2 (p=0,008). Conclusões: Pacientes pediátricos hospitalizados com doença falciforme infectados pelo SARS-CoV-2 apresentaram mais comorbidades e diminuição da contagem de eosinófilos entre a admissão hospitalar e a última consulta médica.

Background Chronic cough (CC) as the main symptom of eosinophilic esophagitis (EoE), associated or not with other signs of esophageal dysfunction, has been poorly studied. Variables: Epidemiological, demographics, atopic, endoscopic, histological, comorbidities, symptoms, time of evolution, endoscopic phenotype, respiratory function tests. Adherence, response to treatment, complications, and evolution Only 20 patients (5%) with EoE had CC. The mean age:24 years, male 85%. TheResults time of evolution: 5 years. CC: recurrent during the day (never at night), worsened with food intake (during or after), improved when remission EoE and reappeared when active EoE. There were a few severe complications. Conclusion Although the frequency of CC as a symptom in EoE is low, it must be considered, especially if it appears in food intake.

ABSTRACT

Objetivo: Evaluar la correlación del recuento diferencial de eosinófilos con niveles de proteínas totales y fosfatasa alcalina en pacientes con valores normales y elevados de aspartato aminotransferasa pertenecientes a un policlínico de Villa El Salvador en Lima, Perú. Materiales y métodos: Estudio observacional, analítico y correlacional realizado en pacientes de ambos sexos con edades iguales o mayores a 18 años, aparentemente sanos, con niveles normales y elevados de la enzima aspartato aminotransferasa. 279 pacientes cumplieron con los criterios de elegibilidad. Se utilizó la prueba de correlación de Spearman para determinar grado de correlación entre recuento de eosinófilos con fosfatasa alcalina y proteínas totales en pacientes con niveles normales y elevados de aspartato aminotransferasa. Resultados: No hubo correlación entre las variables estudiadas en el grupo con valores de aminotransferasas normales. Sin embargo, en el grupo que presentó valores de aspartato aminotransferasa elevados se encontró una correlación moderada y negativa entre el recuento de eosinófilos con las proteínas totales (Rho = -465) y correlación baja y positiva con la fosfatasa alcalina (Rho = 296). Conclusiones: En presencia de valores de aminotransferasas superiores al rango normal, los eosinófilos se correlacionan con las proteínas totales y la fosfatasa alcalina. Son necesarios estudios con mayor número de pacientes y complejidad metodológica para determinar si la interacción entre eosinófilos con la albúmina y la fosfatasa ocurre frecuentemente en condiciones de aminotransferasas elevadas, y considerar posibles implicaciones en la fisiopatología de las enfermedades crónicas asociadas a esta enzima, así como posibles repercusiones clínicas y terapéuticas.


Objective: To evaluate the correlation between eosinophil differential count and total protein and alkaline phosphatase levels in patients with normal and high levels of aspartate aminotransferase from a polyclinic in the district of Villa El Salvador in Lima, Peru. Materials and methods: An observational, analytical and correlational study conducted with apparently healthy male and female patients aged 18 years or older, showing normal and high levels of enzyme aspartate aminotransferase. The study included 279 patients who met the eligibility criteria. Spearman's correlation coefficient was used to determine the correlation between eosinophil count and alkaline phosphatase and total protein in patients with normal and high levels of aspartate aminotransferase. Results: No correlation was found between the variables studied in the group with normal levels of aminotransferases. However, in the group with high levels of aspartate aminotransferase, a moderate and negative correlation was found between eosinophil count and total protein (Rho = -465) and a low and positive correlation between eosinophil count and alkaline phosphatase (Rho = 296). Conclusions: In the presence of aminotransferase levels above the normal range, eosinophils correlate with total protein and alkaline phosphatase. Further studies with a larger number of patients and greater methodological complexity are necessary to determine if the interaction between eosinophils and albumin and phosphatase is frequently seen with high levels of aminotransferases. Moreover, they are necessary to consider possible implications in the pathophysiology of chronic diseases associated with this enzyme, as well as possible clinical and therapeutic implications.

ABSTRACT

Objetivo Relacionar la parasitosis intestinal con los parámetros bioquímicos y hematológicos en niños de la comunidad de Las Trincheras. Métodos Se trató de un estudio de tipo descriptivo-correlacional y diseño de campo, transversal, comparativo en 31 niños seleccionados de forma intencional. Se realizó análisis coproparasitológico directo con solución salina, lugol y Kato. Las proteínas totales y albúmina se determinaron por método colorimétrico. Los parámetros hematológicos fueron evaluados por método manual y el grado de eosinofilia se calculó con base en el valor absoluto de eosinófilos. Resultados El 79,5% (31/39) de los niños estudiados presentaron una o más especies de parásitos con predominio de Blastocystis spp. y Endolimax nana en un 74,2%. Ambos sexos fueron afectados por igual (p>0,05) y mostraron un estado nutricional normal en relación con el IMC/E, principalmente en el grupo de edad de 7 a 10 años. Los valores de proteínas totales, albúmina, hemoglobina, hematocrito y leucocitos fueron normales en la mayoría de los niños. La neutrofilia y linfopenia fue una condición observada en el 45,2% de los poliparasitados mientras que la eosinofilia leve (19,4%) fue mucho más frecuente en los niños monoparasitados. Conclusiones En la muestra de estudio no existe asociación estadísticamente significativa entre los parámetros hematológicos, bioquímicos y la presencia de parasitosis intestinal, no obstante, las variaciones hematológicas, principalmente neutrofilia, linfopenia y eosinofilia leve presentes en los niños poliparasitados por geohelmintos, son un factor asociado a la helmintiasis intestinal y podrían indicar hábitos de fecalismo dentro de la comunidad.


Objective To relate intestinal parasitosis with biochemical and hematological parameters in children from Las Trincheras community. Methods This was a descriptive-correlational study and cross-sectional, comparative field design in 31 children intentionally selected. Direct coproparasitological analysis was performed with saline, lugol and Kato. Total proteins and albumin were determined by colorimetric method. Hematological parameters were evaluated by manual method and the degree of eosinophilia was calculated based on the absolute value of eosinophils. Results 79.5% (31/39) of the children studied presented one or more species of parasites with a predominance of Blastocystis spp. and Endolimax nana in 74.2%. Both sexes were affected equally (p>0.05) and showed a normal nutritional estatus in relation to BMI/A, mainly in the age group of 7 to 10 years. Total protein, albumin, hemoglobin, hematocrit and leukocyte values were normal in most children. Neutrophilia and lymphopenia were an observed condition in 45.2% of polyparasitic, while mild eosinophilia (19.4%) was much more frequent in monoparasitic children. Conclusions In the study sample there is not statistically significant association between hematological and biochemical parameters and the presence of intestinal parasitosis, despite the hematological variations, mainly neutrophilia, lymphopenia, and mild eosinophilia present in children who are polyparasitic by geohelminths, are a factor associated with intestinal helminthiasis and could indicate fecal habits within the community.

Abstract Objective: To perform a quantitative assessment of bronchial wall thickening and the emphysema score in patients with stable chronic obstructive pulmonary disease (COPD), comparing the eosinophilic and non-eosinophilic COPD phenotypes. Materials and Methods: This was a retrospective observational study of patients with COPD followed between August 2018 and July 2019. The patients were divided into two groups by the eosinophil count in peripheral blood: eosinophilic (≥ 300 cells/µL); and non-eosinophilic (< 300 cells/µL). Quantitative, automated assessments of emphysema and bronchial wall thickness were performed by evaluating computed tomography scans of the chest. Results: We evaluated the records of 110 patients diagnosed with COPD: 28 (25.5%) in the eosinophilic group; and 82 (74.5%) in the non-eosinophilic group. The demographic, clinical, functional, and therapeutic variables were comparable between the two groups. There were no significant differences between the two groups in terms of the emphysema score or bronchial wall thickness (p > 0.05 for both). Conclusion: Patients with eosinophilic COPD do not appear to have lower emphysema scores or greater bronchial wall thickening than do those with non-eosinophilic phenotypes of the disease.


Resumo Objetivo: Avaliar quantitativamente o escore de enfisema e o espessamento da parede brônquica de pacientes com doença pulmonar obstrutiva crônica (DPOC) estável e comparar os fenótipos eosinofílico e não eosinofílico. Materiais e Métodos: Estudo observacional, transversal, retrospectivo, que avaliou pacientes com DPOC no período de agosto de 2018 a julho de 2019. Os pacientes foram separados dois grupos, de acordo com o número de eosinófilos periféricos: os eosinofílicos (≥ 300 células/µL) e os não eosinofílicos (< 300 células/µL). Foram realizadas avaliações quantitativas e automatizadas de enfisema e de espessamento brônquico para os dois grupos por meio de tomografia computadorizada de tórax. Resultados: Foram coletados dados de 110 pacientes com o diagnóstico de DPOC, dos quais 28 (25,5%) apresentaram perfil eosinofílico. As variáveis demográficas, clínicas, funcionais e terapêuticas do grupo dos pacientes com perfil eosinofílico foram semelhantes às do grupo não eosinofílico. Não se observaram diferenças significativas em relação ao escore de enfisema e à medida de espessura de parede brônquica entre os dois grupos (p > 0,05). Conclusão: Neste estudo, os pacientes com fenótipo eosinofílico não apresentaram menor escore de enfisema e nem maior espessamento parietal brônquico.

INTRODUCCIÓN: La colitis eosinofílica y la colitis de la enfermedad inflamatoria intestinal, son dos entidades que pueden compartir similares características clínicas, endoscópicas y terapéuticas pero diferentes criterios diagnósticos. OBJETIVOS: Describir el caso clínico de un niño preescolar con antecedente de alergia alimentaria, de hospitalizaciones y uso de antibióticos por varias ocasiones, que evoluciona con diarrea crónica intermitente. CASO CLÍNICO: Se trata de un paciente masculino, de 3 años 5 meses, con antecedente de alergia alimentaria con cuadro crónico de dolor abdominal, diarrea y retraso en el crecimiento. Se realiza abordaje de diarrea crónica. RESULTADOS: Con hallazgos clínicos de enfermedad inflamatoria intestinal y descripción histopatológica de colitis eosinofílica, se considera la asociación entre estas dos patologías sin dejar la posibilidad de que esta última se trate de una fase inicial de enfermedad inflamatoria intestinal. CONCLUSIONES: El tratamiento de pacientes con colitis eosinofílica complicada es similar a la enfermedad inflamatoria intestinal, se requiere seguimiento clínico, endoscópico e histopatológico de pacientes con colitis eosinofílica a largo plazo.


INTRODUCTION: Eosinophilic colitis and inflammatory bowel disease colitis are two entities that may share similar clinical, endoscopic and therapeutic features but different diagnostic criteria. OBJECTIVES: To describe the clinical case of a preschool child with a history of food allergy, hospitalizations and use of antibiotics for several occasions, who evolves with chronic intermittent diarrhea. CLINICAL CASE: This is a male patient, 3 years 5 months old, with a history of food allergy with chronic abdominal pain, diarrhea and growth retardation. Chronic diarrhea was approached. RESULTS: With clinical findings of inflammatory bowel disease and histopathological description of eosinophilic colitis, the association between these two pathologies is considered without leaving the possibility that the latter is an initial phase of inflammatory bowel disease. CONCLUSIONS: The treatment of patients with complicated eosinophilic colitis is similar to inflammatory bowel disease, clinical, endoscopic and histopathological follow-up of patients with eosinophilic colitis is required in the long term.

ABSTRACT

The eosinophil is a cell of the immune system, with an arsenal of substances that can alter the balance that exists in the different organs where they are found. With the advent of monoclonal antibodies, concern about their depletion has become an important turning point in their formulation. For this reason, it is of vital importance to investigate the consequences of the mechanism of action of biological agents, in the short and long term. This review tries to show the role of eosinophils in both homeostasis and disease, and their relationship and interaction with monoclonal drugs in diseases focused on the Th2 profile. It is expected that this article can be useful when making the decision to start treatment with monoclonals, specifically anti-interleukin-5 or against its receptor

【Objective】 To study the value of neutrophil/lymphocyte ratio (NLR) and platelet/lymphocyte ratio (PLR) in the diagnosis and prediction of disease severity in adolescent atopic dermatitis (AD). 【Methods】 A total of 100 AD patients and 50 healthy individuals were selected from Dermatology Department of our hospital.The age, gender, blood routine results of all subjects were collected, and NLR and PLR were calculated.The diagnostic value of NLR and PLR for AD and severe AD was discussed by receiver operating curve (ROC), the relationship between NLR, PLR and the risk of AD was analyzed. 【Results】 The AUC of NLR and PLR in the diagnosis of AD was 0.780 and 0.769, with the best cutoff value at 2.95 and 98.50 respectively, and no significant difference in the predictive value between NLR and PLR was noticed (P>0.05). The proportion of NLR≥2.95, PLR≥98.50, red blood cell distribution width (RDW), mean platelet volume (MPV), white blood cell count (WBC) and eosinophils (E) in AD group were significantly higher than those in control group (all P<0.05). Before and after adjusting for confounding factors, the odds ratio (OR) of patients with NLR≥2.95 and PLR≥98.50 were 12.250 vs 6.048 and 5.525 vs 4.352, respectively.NLR, PLR and E in mild AD group were lower than those in severe AD group, and NLR and E in moderate AD group were lower than those in severe AD group, the differences were statistically significant (all P<0.05). The AUC of NLR and PLR in diagnosis of severe AD was respectively 0.712 and 0.675, the difference was statistically significant (P<0.05). NLR, PLR, RDW and E were positively correlated with SCORAD score, and the r values were 0.254, 0.198, 0.202 and 0.219 respectively. 【Conclusion】 The increase of NLR and PLR were the risk factors of AD, and had certain diagnostic value for AD and severe AD.

@#AIM: To investigate the value of conjunctival impression cytology(CIC)in the diagnosis of allergic conjunctivitis(AC). <p>METHODS: A case-control study. Forty patients(80 eyes)who were clinically diagnosed with AC in our department from January to February 2021 were included in the study,and 40 normal controls(80 eyes)were enrolled too. All subjects underwent double upper palpebral CIC examinations. Observed the morphology and number of conjunctival eosinophils and carried out Tseng classification in each group. To Compare the differences between the results of the two groups and analyze the effects of gender, age and course of disease on the results of CIC eosinophil examination in AC group.<p>RESULTS: The positive rate of eosinophils in the upper palpebral CIC examination of AC patients was significantly higher than that of the normal control group(χ<sup>2</sup>=57.744, <i>P</i><0.001); The Tseng grade of AC patients was also higher than that of the normal control group(<i>Z</i>=-2.453, <i>P</i><0.05). Gender, age and course of disease had no significant effect on the results of CIC eosinophils in AC group(all <i>P</i> >0.05).<p>CONCLUSION: CIC examination is simple, and the result is objective and accurate. It has good application value in the diagnosis of AC.

ABSTRACT

The clinical data of 4 patients with acute eosinophilic pneumonia (AEP) diagnosed in the Department of Respiratory and Critical Care Medicine, Peking University Third Hospital were retrospectively analyzed. All 4 patients were males, aged 28, 34, 43 and 68 years respectively. The main manifestations were fever, cough and dyspnea. Three cases had a history of smoking and 1 case had a history of ulcerative colitis. The proportion of eosinophils in bronchoalveolar lavage fluid (BALF) was 85%, 93%, 41% and 50%, respectively. The proportion of eosinophils in peripheral blood was 53.3% (16.29×10 9/L), 25.1% (1.29×10 9/L), 4.8% (0.4×10 9/L) and 13.7% (1.55×10 9/L), respectively. Blood gas analysis (without supplemental oxygen) showed type I respiratory failure in 1 case, hypoxemia in 3 cases. The total IgE (normal range ≤ 100.0 KU/L) in peripheral blood of the 4 cases was>2 500.0 KU/L, 60.3 KU/L, 379.4 KU/L and 407.0 KU/L, respectively. HRCT showed diffuse distribution of lesions in both lungs, including ground glass opacity, patchy consolidation and centrilobular nodules. After diagnosis, all patients were treated with systemic glucocorticoids. Fever, cough and dyspnea, as well as the chest CT lesions were significantly improved after treatment. The patients were followed up for 29 months, 16 months, 18 months and 24 months respectively, without recurrence. AEP is a rare disease which is easy to be misdiagnosed as severe pneumonia and/or ARDS. Eosinophilia in peripheral blood is suggestive of the disease, and timely BALF cell differentials is important for early diagnosis.
